Position Summary
To facilitate the accurate and timely tasks of collecting and imaging medical records, performing deficiency analysis assisting with release of information requests, MRN merging and processing birth certificates as required by law.  


Position Requirements

  • High School Graduate or G.E.D.

Workplace Experience

  • Experience in hospital Health Information Management Department or completion of a certified Health Information Technology or Management Program.
  • Excellent communication skills.
  • Excellent customer service experience and skills.

Equipment and Skills Training

  • Computer skills required
  • Good number-letter recognition skills.

Physical Effort

  • Prolonged sitting, repetitive motion i.e. keyboarding, bending, reaching, and stretching to retrieve items up to 15 pounds.
  • Prolonged viewing of computer screens and charts consisting of typed and handwritten entries.


Key Results

  • Performs batch ‘prep’ functions to ensure patient charts/documents are ready to be scanned and indexed into Meditech.
  • Performs scanning functions on documents while maintaining integrity of batch assembly and monitoring images for completeness and legibility. 
  • Performs verification and indexing of images for quality completeness and account/document identification.
  • Researches and merges duplicate medical records numbers. 
  • Moves scanned medical record documents associated with the wrong patient, wrong account or wrong document type.
  • Performs accurate deficiency analysis for all patient types according to established standards to ensure compliance with The Joint Commission, CMS and other Regulatory agencies.  
  • Meets turn-around-time standards for processing of charts from discharge to availability in Electronic Medical Record system.
  • Meets quality standards for prepping, scanning and indexing medical record documents for all patient types. 
  • Retrieves medical record documents from hospital patient care areas and prepares for transit to HIM Department for processing.
  • Scans Release of Information requests to vendor for processing and coordinates retrieval and return of medical records from offsite storage vendor when needed. 
  • Assists with internal release of information requests including PFS and Risk Management.
  • Provides back up for Birth Certificate Coordinator/Patient Portal as assigned.
